Long Term Care

We offer long-term care which can involve a variety of services to help support/maintain an individual with both medical and non-medical needs of people who have been diagnosed with a chronic illness, acquired brain injury, or disability and are now unable to care for themselves for long periods of time.

Our long-term care is focused on working with other health professionals to ensure that we promote independence, we give the individual the best quality of life, and to meet the individual’s outcomes to enable them to remain in their own home.
